Two ways to register - by mail or in person
by mail:
To register your student for camp, print and complete the student health f0rm, and the waiver;(please write your email address at the top of the student health form).
If you will be sending doctor prescribed medication to camp, print and complete the medical authorization form, the special meals request form requires a doctor's signature also. There are links to all the forms at the bottom of this page. Forms are in both Spanish and English.

You can send a check, money order, or cashiers check with your paperwork. (Deposit of $225. will hold a spot, balance payment of $150.00 due by September 28th. Total cost of 375.00 by check) make checks payable to Outdoor Opportunity Group One. Or, pay online. Please see the Pay For Camp page for online payment.
